2. Hospital did not have detailed list of nurses nor had duty rosters of nursing staff at any place including high risk areas like ICU, HDU, A&E, Labor room later provider list of 56 nurses
3. No evidence of Nurse In-charge
4. The hospital submitted a list of consultants but on day of inspection, only few were seen and manpower list submitted did not match with actual.
5. There was no separate OPD register for each OPD to verify the doctors/consultants and patients.
6. Labor room was very congested. The labor room had double entry not secluded /safe space as one entry was from the corridor directly,
7. There was no evidence of patient monitoring in labor room.
8. There was no hand grip in toilet.
9. No evidence of emergency medication.
10. No neonatologist was evidenced.
11. No evidence of appropriately qualified staff to manage the ICU and Emergency. HDU/ICU/Emergency is managed by BAMS doctors.
12. There was no evidence of external and internal Quality Assurance and calibration of machines in Laboratory.
13. As there was no evidence/ process for computerized registration of the patient, but later hospital produces the computerized OPD registration.
