2.AERB for CT is not available. No TLD badge or lead apron available in hospital.
3.Blood bank no doctors available, Lab no full time consultant available (no external quality control being followed).
4.Bio medical waste rules adherence is not their, ETP plant is not their as per state PCB rules.
5.Kitchen and canteen are situated in 2nd basement of hospital. As it is a fire hazard and also firefighting mechanism is not implemented in hospital.
6.DMO duty medical officer (9) are lesser than IPHS guidelines i.e. 11 per 100 beds.
7.No defibrillator in Emergency room.
8.Non availability of post-operative rooms in OT.
9.MTP registration is not available.
10.Poor hygienic conditions, no infection control measures are taken on regular bases.
11.Assessors report is attached with recommendation.
