1.Inadequate no. of qualified (GNM or above) nursing staff as per IPHS guidelines: On the day of inspection only 18 nurses were available for a bed strength of 135 as per the list of manpower submitted.
2.For specialities like OBG, Dental and Surgery, qualified doctors were not available.
3.For Orthopaedics, a MD qualified physician is shown as surgeon.
4.PCB consent to generate BMW was not evidenced.
5.Fire NOC from respective authorities was expired.
