•Proper sterility of OT is not up to the mark, as documents of fumigation are not maintained and microbiological surveillance are not conducted at regular interval.
•BMW rules 2016 are not followed and implemented.
•Records of OT are not maintained.
•All major equipment’s are installed in one room without keeping the privacy of patients and causing space constrained.
•Seepage of water logging in room where ophthalmology equipment’s are installed. Water logging in basement.
•Lack of awareness about fire safety among the staff for installed two fire extinguishers.
