• The hospital has failed to justify reduction in number of beds. As per HCO’s application and assessor’s report the number of beds is 100 whereas the mail received from the HCO dated 18-08-2022states that HCO is 45 bedded hospital.
• The hospital is only certified by NABH entry level and not accredited by NABH as claimed by HCO.
• Guidelines and eligibility criteria for Entry Level certification and CGHS empanelment are different and the same has been informed to the HCO multiple times, So HCO is required to comply as per CGHS empanelment criteria.
• The hospital needs to have RMO's and nurses as per sanctioned bed strength which has been declared as 100 in the application form. Sudden reduction of bed detail from 100 to 45 cannot be accepted. Since the hospital has refused to employ 11 RMO's and 45 nurses according to the sanctioned bed strength of 100 as per IPHS Standards and hospital is functional with 4 RMO’s and 29 Nurses.
Despite repeated reminders to employ required number of RMO’s and nurses as per sanctioned and operational bed strength HCO has refused to employ RMO’s as per last communication i.e. 9 RMO’s and Nurses.

• Non – availability of demarcated area in CSSD for Receiving, Cleaning, Packing & Storage.
• No temperature records for refrigerator inside the pharmacy.
