1.No evidence was found of MOU with BMW collecting Agency for disposal of Bio-medical waste.
2.In the Certificate for Generation from Haryana Pollution Control Board the address of the HCO was mentioned as House Number 25, whereas in other documents it was written as House Number 27.
3.The MOU with the MDS specialities were on plain A4 paper with no start date or end date.
4.The specialities MDS of Prosthodontics, Conservative & Endodontics, Periodontics, Orthodontics & Dentofacial Orthopaedics, Pedodontics and Preventive Dentistry, have not seen even a single patient at the clinic since the clinic started.
5.There Degrees were not there with the HCO, also HCO did not have the Prosthodontics, Conservative & Endodontics, Periodontics, Orthodontics & Dentofacial Orthopaedics, Pedodontics and Preventive Dentistry.
6.At the entrance door the HCO had put a sticker— “CGHS, ESI & ECHS APPROVED”, but their other branch was recognized with these institutions.
7.There was no tariff list.
8.There was no reception at the HCO, the Dental assistant cum receptionist sat opposite the doctor & fills the initial assessment form cum registration form. Sometimes they register in “PRACTO” Software sometimes on physical registration form. There was no uniformity.
9.There was no SOP for record keeping neither are being kept properly.
10.The patients were given the receipts only when they demand. That too sometime through PRACTO software, & sometimes through hard copy of receipt.
11.On asking for the receipt of one of the patient, the HCO was unable to find the receipt book, then after 10 minutes showed receipt number 5425 for the patient who came on 04-11-2020. But receipt number 5424 was dated 09-11-2020 & there was no patient on 5426. This clearly proved that they had made the receipt during the inspection time only whereas the previous receipt number 5424 was of 09-11-2020 patient.
12.The receipt which was produced by the software “PRACTO” had name of “MUSKAN DENTAL” on top of it; which according to HCO is there second branch and not the one which they are going in for CGHS Inspection.
13.The HCO had a pressure cooker type autoclave which only had pressure gauge. Secondly there was no designated washing area, cleaning of instrument area, packaging area. Even the Pressure cooker type autoclave was kept on the floor. No Biological or Chemical Indicator are being used. No calibration of the Autoclave.
14.HCO had 1 wash basin which is approx. 18” in size which was also used to wash hands and it was evident that the instrument washing cannot be done there properly.
15.Personal files of the staff were not being maintained properly.
16.The HCO could not provide any record for Prophylaxis for Hepatitis for Dr. Vandana nor for Ms. Mampi Mahanta.
17.No proof of availability of Personal Protective Devices (PPD) like Lead Apron, Thyroid Collar, Screen etc. was with the HCO.
18.There was no Portable/ Drinking water at the HCO for the patient.
19.There was no Fire Extinguisher at the HCO.
20.The address of HCO in Certificate obtained from Haryana Pollution Control Board for Generation of BMW Waste was different, also in the MOU’s with the MDS Doctors its different BUT in all other documents including Labour Department registration, Fire NOC & AERB Certificate it was different
